• CONSENT TO RELEASE FORM
  • Authorizes release of confidential immigration, academic, or personal information to designated individuals.
• COURSE SELECTION FORM
  • Allows exchange students to list seven preferred courses (ranked) for registration in five; first-year undergraduates are placed by the Registrar, and degree students register via their advisor.
• CPT REQUEST FORM
  
  • For F-1 students requesting Curricular Practical Training, including employment details, academic relevance, and advisor approval.

• PARENT/GUARDIAN CONSENT FOR MINORS

  • Allows F-1/J-1 students to take fewer credits than full-time enrollment for approved academic or medical reasons.

• RELEASE OF INFORMATION FORM
  • Authorizes the office to share student records with approved third parties.
• REPLACEMENT I-20/DS-2019 FORM
  • Requests a new I-20 or DS-2019 if the original document is lost, stolen, or damaged.
• SACM LETTER REQUEST FORM
  
  • Used by SACM-sponsored students to request official letters such as change of major or scholarship extension.
• TRANSFER IN FORM
  
  • Used to transfer an active SEVIS record from another U.S. school to Millersville.
• TRANSFER OUT FORM
  • Notifies the office that a student intends to transfer their SEVIS record to another U.S. institution.

  • Used to request corrections or updates (name, address, major, etc.) on immigration documents.

  • For returning students to complete SEVIS registration (opens January 5, 2026); in-person check-in required at Lyle Hall.
• SSN Letter Request: On or Off Campus Employment Verification (Student)
  • Requests a letter confirming eligibility for a Social Security Number based on authorized employment.

  • Submitted before the Transfer Out Form to provide feedback on your Millersville experience.
• Source of Funds Form (must log into your application form)
  • Required financial documentation for I-20/DS-2019 issuance, completed in the application portal.
• Travel Signature Request
  • An international student needs a travel signature to confirm their SEVIS record is active and to be allowed reentry into the United States after traveling abroad. Signatures are valid for 12 months.
